What a mold exposure test actually measures
The phrase “mold exposure test” covers several distinct laboratory analyses that measure different biological signals. Some measure the body’s immune response to mold antigens. Others attempt to detect mold-produced toxic compounds (mycotoxins) in body fluids. Still others measure inflammatory markers that can be associated with mold exposure but are not specific to it. Each test category answers a different question, and a physician selects from the menu based on the patient’s symptoms, history, and the suspected exposure pathway.
No single test confirms “mold illness” as a unified diagnosis. The medical literature is still actively debating which biomarkers correlate reliably with environmental mold exposure and at what thresholds. Patients and homeowners should treat any single positive result as one data point in a clinical picture rather than as a standalone diagnosis. CDC, EPA, and the American Academy of Allergy, Asthma, and Immunology all recommend interpreting these tests in the context of a full clinical evaluation.
IgE blood tests for mold allergy
Specific IgE blood testing is the most clinically established mold-related test. Immunoglobulin E (IgE) is the antibody class that drives allergic reactions. A specific IgE panel for mold typically measures antibodies against several common environmental mold genera: Alternaria, Cladosporium, Aspergillus, Penicillium, and others. The lab reports each antibody level in kilounits per liter, with reference ranges that flag elevated responses.
A positive specific IgE for a particular mold genus means the patient has been sensitized to that mold and may experience allergic symptoms on exposure. It does not necessarily mean the patient is being actively exposed, and it does not measure the severity of any symptoms. Physicians often combine specific IgE results with the patient’s symptom history and a skin-prick test to build a clinical picture. NIH allergy literature treats specific IgE as a useful screen for mold allergy in the context of asthma, allergic rhinitis, and atopic dermatitis.
Skin-prick allergy testing
Skin-prick testing is the in-office counterpart to specific IgE blood testing. A small amount of mold antigen extract is placed on the skin, the skin is gently pricked, and the response is read after 15 to 20 minutes. A wheal-and-flare reaction larger than a control prick indicates sensitization. Allergists often prefer skin-prick testing because it gives immediate results and tests multiple antigens in one visit. Patients on antihistamines are typically asked to stop them for several days before the test, since the medication suppresses the skin response.
IgG blood tests and their controversy
Specific IgG testing for mold is offered by many commercial labs but its clinical interpretation is much more contested. IgG is an antibody class that develops as part of normal immune memory after exposure to almost any antigen. A positive IgG for a mold genus typically means the patient has been exposed to that mold at some point, but exposure is essentially universal: everyone breathes in mold spores from outdoor air every day, so almost everyone will develop some IgG response to common mold species.
The American Academy of Allergy, Asthma, and Immunology has issued position statements noting that mold IgG testing has limited clinical utility for diagnosing mold-related illness in most patients. Some functional medicine practitioners use IgG panels as part of a broader assessment, but the standard allergy and immunology community treats IgG mold panels as informational rather than diagnostic. Patients should ask their physician how the result will be interpreted before paying for the test.
Urine mycotoxin testing
Urine mycotoxin testing measures trace amounts of fungal toxins (ochratoxin A, aflatoxins, trichothecenes, and others) in urine. The premise is that mold-produced toxins absorbed through inhalation or ingestion are excreted through urine and can be detected by mass spectrometry. Several commercial labs offer these panels and they are commonly ordered by integrative and functional medicine practitioners working with patients who suspect mold exposure.
The scientific literature on urine mycotoxin testing is mixed. CDC and mainstream environmental medicine groups note that mycotoxins in food (particularly grains, coffee, and dried fruit) are a known source of detectable urinary mycotoxins independent of any environmental mold exposure, which makes interpreting a positive result difficult. The thresholds used by commercial labs for what counts as “elevated” are not standardized across providers. Patients considering this testing should discuss with their physician what action would be taken on a positive result before ordering.
Inflammatory and complement markers
Several non-specific inflammatory markers are sometimes ordered as part of an environmental medicine workup. These include high-sensitivity C-reactive protein, transforming growth factor beta-1, matrix metalloproteinase-9, vascular endothelial growth factor, and the complement components C3a and C4a. None of these markers is specific to mold exposure. Each can be elevated in many inflammatory conditions including infection, autoimmune disease, and chronic stress.
The clinical use of these markers in mold-related illness is most associated with the Shoemaker chronic inflammatory response syndrome (CIRS) protocol, which remains controversial in mainstream medicine. Patients exploring this framework should work with a physician familiar with both the protocol and its critics so that interpretation is balanced.
Pulmonary function testing for occupational exposure
For patients with suspected occupational mold exposure (agricultural workers, demolition crews, remediation workers, water-damage restoration technicians), pulmonary function testing is often part of the workup. Spirometry measures lung capacity and airflow. Methacholine challenge testing can identify airway hyperreactivity consistent with occupational asthma. Hypersensitivity pneumonitis panels measure IgG against specific occupational antigens including thermophilic actinomycetes and certain mold species.
OSHA recognizes mold-related hypersensitivity pneumonitis as an occupational illness in specific industries. Patients with high-intensity occupational exposure should ask whether their physician is comfortable with the occupational medicine literature or whether a referral to a pulmonologist or occupational medicine specialist is warranted.
What test results do and do not prove
A positive specific IgE for a particular mold genus proves sensitization, not active exposure. A positive IgG proves prior contact with the antigen at some point, which is nearly universal. A positive urine mycotoxin result is open to interpretation depending on dietary exposure and the lab’s reference range. A normal pulmonary function test does not rule out mild reactive airway disease that has been compensated. The clinical picture, including symptom timing relative to home or workplace exposure, is at least as important as any single lab number.
Conversely, negative results across all the panels do not rule out a problem. Mold-related illness, where it exists, often shows up as a constellation of symptoms rather than a single diagnostic signature. The patient’s physician is the right person to weigh these results against the rest of the clinical picture.
When environmental testing fits alongside medical testing
Many patients who pursue medical mold testing also commission an environmental mold inspection of their home or workplace. The two investigations answer different questions and rarely substitute for each other. The medical test asks: is this person reacting to mold antigens. The environmental test asks: is there mold in this building. A positive medical test plus a negative environmental test points toward an exposure somewhere outside the home or to historical exposure. A negative medical test plus a positive environmental test still warrants source remediation because of long-term exposure risk to other occupants.
